A Beacon of Quality Care!
The Centers for Medicare & Medicaid Services (CMS) created the Overall Hospital Star Rating program to help patients and their loved ones compare hospitals.
The federal agency assigns star ratings to more than 4,500 hospitals nationwide based on their performance across seven quality performance areas: mortality, safety of care, readmission, patient experience, effectiveness of care, timeliness of care and efficient use of medical imaging. Typically, less than 10% of facilities nationwide earn such a designation.
To provide superior clinical care, rehabilitation, wellness, and supportive services that meet the wants, needs, and expectations of our patients and residents – while achieving “quadruple aim” goals of better individual health care, better population health, lower costs and improved clinician experience – and drive performance and outcomes that are consistent with the demands of acute care partners and payor organizations.
To provide integrated post-acute health care, accountable care, services and technology that reduce rehospitalizations, generate quality clinical outcomes and customer service, navigate patients cost-effectively through post-acute care, and reduce costly outliers for providers, while developing industry-leading programs and capabilities and growing relationships with strategic partners.
A long-term care ombudsman helps residents of a nursing facility and residents of an assisted living facility resolve complaints. Help provided by an ombudsman is confidential and free of charge. To speak with an ombudsman, a person may call the toll-free number 806-795-5433 or email firstname.lastname@example.org.
